Injuries/Illnesses at School
The health office will contact you if your child is ill. Children will be sent home if the following symptoms are present: fever of 100 degrees or greater, vomiting, questionable skin rash, discharge from the eyes, or severe cough. Students may not return to school until they are fever-free and have not vomited for 24 hours.  More information...

Medication at School
The nurse may administer medications necessary to maintain a healthy child in school only if an authorization form has been signed by both the physician and the parent. This includes both prescription and over the counter medications. The school health offices do not supply any medication. Only labeled prescription bottles or medication in their original packaging will be accepted.

Asthma inhalers and epipens are the only medications permitted by Illinois law which a student may self carry. Parental written permission is required along with the prescription box for an inhaler to be self carried by a student.

Allergy Guidelines
Please notify your school nurse if your child has any food allergies or dietary restrictions. All life-threatening food allergies coming forward to the school’s attention will initiate an Emergency Action Plan (EAP) personalized for each child. All Individual Health Care Plans (IHP) or 504 Plans will include an EAP for students with life threatening allergies.

Physical Education
All students are expected to participate in physical education classes. If a student is unable to take part for health reasons, a written note from the parent may excuse the student for up to three days from gym. Should it be necessary to be excused for longer than three days, a doctor’s note is required.

Absences or Late Arrivals
All absences and late arrivals must be called in to the school attendance number by a parent prior to a child’s absence. This is an automated phone system available 24 hours a day. Please leave a message if your child will be absent or late.

An emergency treatment/heath form is required every year, with parent signature and date, submitted prior to the start of school.

Physical Examination
Illinois Department of Public Health requires a completed exam within one year prior to the date of entrance for all students transferring from out-of-state schools as well as those entering the following grade level/programs: Early Childhood, Kindergarten, First Grade only if this is the first entry into school, and Sixth Grade. This exam should be submitted prior to the start of the school year.

Sports Physical Examination for Athletic Participants at AJHS (5th through 8th grade)
A current sports physical must be on file in the school nurse's office prior to participating in any after school sport sponsored by AJHS and the IL Elementary School Association (IESA).
This includes any try-outs, practices, or pre-season activities.

Vision Examination
A vision exam is required for all students entering Kindergarten or new students entering IL for the first time from out of state or out of country. The exam is due by October 15 of the school year and must be completed by a licensed eye care professional.
